Since 2021 the Department of Land Transport has required a medical certificate both for new driving licences and for renewals, so we are often asked 'which certificate do I need for my licence?' It is the Thai Medical Council form, which any licensed medical clinic can issue, and in most cases you can wait for it.

Which certificate is required
The Department of Land Transport requires the certificate approved by the Thai Medical Council. It has two parts: the applicant's own health declaration, and the doctor's examination and opinion. The doctor must certify that you have none of the five prohibited diseases under the ministerial regulation and no disease or condition that would impair driving.
Watch the document's age: it must have been issued no more than 1 month before the day you apply. If you get examined too far in advance you will need a new one, so fix your transport-office date first, then come for the examination.
What the doctor checks
- History of conditions that affect driving, such as epilepsy, uncontrolled heart disease, diabetes with frequent low blood sugar, or eye disease
- Blood pressure, pulse, weight and height
- A general physical examination, checking for signs of the five prohibited diseases

Good to know: A standard driving-licence certificate needs no blood test and no fasting. Colour-vision, eyesight and reaction tests are done at the transport office, not as part of the certificate.
Documents for the transport office
For renewing a private licence the main documents are your original ID card (or passport with a valid visa and work permit or residence certificate for foreign nationals), your current licence and a medical certificate under 1 month old. Renewing a 5-year licence to another 5 years requires training, now available in advance through the DLT e-Learning system, plus physical tests at the office.
